2) To qualify for this offer, you must include:
• Original or copy of invoice(s), receipt(s), or packing list(s) dated during the Promotion Period with the purchase
price(s) circled for each qualifying product purchased.
• Security seal/hologram proof of purchase from Original HP Toner product box for each qualifying product purchased.
• Original UPC proof of purchase from Original HP Ink product box for each qualifying product purchased.
• If mailing, you must complete a separate mail-in form for each invoice submitted and place each in a separate envelope.
• Request must be submitted via the URL within 30 days of purchase or mail-in form(s) must be postmarked within
30 days of purchase.
• Once received and processed (allow 8–10 weeks for processing), HP will mail the gift card to you.

Terms and Conditions
• Cartridges must be new and listed on the qualifying SKU chart. Offer is not valid for refurbished or demo HP products.
• Product(s) purchased before or after the Promotion Period are not eligible for this offer. Purchase date is determined by
invoice or receipt date.
• Offer is limited to product on hand and no substitutions with other products are allowed.
• Only purchases made by an end-user customer or a commercial business from an authorized HP reseller in the U.S. are
valid. Purchases by HP resellers and internal HP orders are ineligible.
• Keep copies of materials submitted. Originals become HP property and will not be returned.
• Claims postmarked or submitted after applicable dates and incomplete or illegible submissions will be returned as ineligible.
• Data received by HP will only be used to confirm proof of eligibility. HP holds such information in accordance with HP’s
privacy policy at welcome.hp.com/country/us/en/privacy.html.
• If terms and conditions are not met, rebate will not be sent.
• This offer may NOT be combined with any other toner or ink cartridge offers. This offer may be combined with HP or
reseller hardware offers.
• HP is not responsible for any claims lost, damaged or delayed in transit. Rebate will not be reissued.
